Please read the Short Term Leave of Absence policy carefully before submitting this form. The short-term leave of absence request form should be completed if you are experiencing a qualifying event (see “Reason for Absence” below), and require an absence from classes for more than two days but no more than ten days. (If you need two days or less, please contact your instructors directly to request an excused absence.)
